Client Agreement Form

This form is used to formalize the agreement between client and psychologist. It includes information regarding our fees for appointments and reports, cancellation policy and confidentiality. There is also a link to the Australian Psychological Societies (APS) Client Charter. The APS Client Charter is a vital part of our Client Agreement Form because it outlines what you have the right to expect from your psychologist.
